Providence Holy Cross Medical Center awarded a $150,000 grant to the San Fernando Community Health Center (SFCHC) to help underserved communities in the San Fernando valley.
SFCHC is a federally qualified health center that offers care to patients regardless of their income or ability to pay. Chief Executive Audrey Simons said the grant “is quite a significant contribution from a community partner.” The grant will make the health center’s case management department more robust and will help expand the case management team.
At least two more health workers and a case manager will be added to the department’s five-member team. The new hires will receive training for patient management risk software. The software enables primary care facilities to identify, organize and refer high-risk patients who could benefit from health services.
“We’re providing the whole circumference of care for the individual – not just medical, not just behavioral health, but referring them to the entire clinic’s portfolio services for the people who are most in need,” Heidi Lennartz, SFCHC’s chief operating officer, said. Simons added that if the health center can minimize how often underserved people are moving around to find quality health services, it would be considered a successful use of the patient management software.
SFCHC works in tandem with Providence Holy Cross Medical Center. SFCHC accepts Providence patients that don’t have a primary care provider but need to see physicians after being discharged. The health center also takes referrals from Providence’s ER.
