The dental PPO network Dental Blue by Thousand Oaks-based Blue Cross of California has become one of the largest PPO networks in the state. The program, from affiliate BC Life and Health Insurance Co., has more than 18,600 provider locations in the state and features three tiers of provider networks. The program has nearly 4,000 dental specialists.
